Do you want the form (in the view), or do you also want the functionality for saving form data, validation etc.?
HTML and JQuery and such are just the same as with normal forms.
In the controller you just have input all. You can do validation within your model.
Use Jeffery Way's Generator and make life a little easier. ;)
